Community Action Network (CAN) Coordinator

Position Summary:
PHS is seeking a Community Action Network (CAN) Coordinator for a HRSA-funded Healthy Start Program in Southeast Queens and surrounding neighborhoods. 
The CAN Coordinator, under the supervision of the Healthy Start Program Director, is responsible for convening key community stakeholders, including but not limited to: consumers, partnering agencies, community-based organizations, and community members to work together to achieve common goals for community betterment. The CAN Coordinator will lead the group in developing yearly strategic plans by gathering and analyzing data to help identify community health needs and assuring we maintain collective impact by encouraging others to integrate and address social determinants of health. 

Duties and Responsibilities: 
• Lead/facilitate bi-monthly CAN meetings virtually or in person  
• Conduct continuous targeted outreach to community partners and local community groups, agencies, and organizations to identify partners and build a cross-sector network of CAN members
• Work with project partners to identify, recruit, and retain program participants to serve on the CAN
• Organize and maintain networks of individuals who support the goals of the Queens Healthy Start Project
• Coordinate trainings for CAN members in skills such as community-based participatory research, data analysis, and asset mapping and on topics such as toxic stress, trauma-informed care, and health insurance enrollment and others the CAN has selected
• Provide guidance and lead the CAN in selecting a common agenda, shared measures, and mutually reinforcing activities
• Plan and organize CAN events such as focus groups, town halls, health and educational sessions
• Work with the Healthy Start local evaluation subcontractor to design and implement evaluation activities for the CAN
• Ensure that data tracking guidelines are met and that program outcomes can be effectively measured
• Draft reports regarding grant activities and program evaluation and outcomes

Qualification Requirements:
• 3+ years of experience in community organizing, strategic planning, and facilitating cross-sector partnerships
• Experience in public health, community development and health issues in urban, underserved neighborhoods
• Ability to be non-judgmental and establish trusting relationships, good problem solving, listening and practical skills; shows respect and sensitivity with individuals of differing cultural, racial, sexual or ethnic identities; some knowledge of community resources. 
• Open to working evening hours and weekends when necessary 
• Bi-lingual preferred (Spanish)
